Membership Resignation Request
The following form should only be filled out if you, and all of those included in your current membership, are resigning your membership. If you are looking to change your membership type (example: upgrade to a dual or family, downgrade to an individual or remove an amenity or service), please visit your club for assistance.
*Did you know: You can place your membership on leave of absence instead of cancelling? This will ensure you keep your monthly rate when you return. For more information click here.
Form submission and response expectation
Upon agreement to the terms and submission of this form, you will receive an automated email response as your confirmation of resignation request. If your submission does not comply with your membership agreement requirements, we will email you within 3 business days with the subject “Membership resignation follow up,” and provide details you must achieve to complete resignation. In this case, your membership will continue as active until you have fulfilled your obligations.
If you do not receive an e-mail with the above subject line within 5 business days, you have completed the resignation requirements. We welcome you to enjoy the club until the final day of your membership (full calendar month after your final billing cycle).
Final notes before filling out this form
If you are requesting resignation of your membership, we are sorry to lose you. We would greatly appreciate your feedback by taking this survey that will take no longer than 2 minutes of your time. Please also recognize that resignation will forfeit your current membership benefits. If you choose to rejoin in the future, you will join at the prevailing rates.
